I just wanted to share the steps I've done to fix the issue and get it cleaned up back to its original luster.
Week 1, spent a couple of days data logging to see if recalibration would fix the issue. After many resets and test drives, there was no improvements and finally the transmission just did not want to play anymore and locked out all gears.
Next was the decision to remove the shift actuator assembly to see what I can find there.
After removal of the whole assembly, I was able to see that there is a damage on the gear position sensor and metal shavings attached to the magnetic sensor casing which would definitely interfere with the proper functioning of the sensor.
Week 2 was spent trying to decide whether to troubleshoot and fix the original assembly and researching if parts are even available, in the end decided to replace the complete assembly. The cost of individual parts and not knowing if there other parts that are bad made me decide to buy the whole assembly instead. I found a company out of Canada that sells individual parts for it called ML Engineering (MLR). I bought the part from MoparPartsGiant.
Weeks 3&4 was spent waiting for the part, cleaning, pressure washing, wet sanding, buffing, interior deep clean, flush and fill gearbox.
